Sample Report Templates, Required Appendices and Instructions

The updated ACEN Guidelines for Written Reports provides a wealth of information to support faculty in writing to the Standards and Criteria. In addition, these guidelines include an optional template for the structure of the report itself. This template can be customized for any report type (i.e., a Self-Study Report, Candidacy Presentation, Follow-Up Report, or a Focused Visit Report). The template incorporates the demographic information needed by your peers, the format for the list of documents and interviewees, each Standard and Criterion statement, as well as data tables, which align with the data tables peer evaluators must complete in their Site Visit Report. Although the use of this template is optional, program leaders and nursing faculty may find the template useful in organizing the written report and ensuring the inclusion of important and required components of each report.

As noted in the guidelines, select appendices are required when the written report includes Standard 2 Faculty, Standard 4 Curriculum, and/or Standard 6 Outcomes. When writing to Standard 2, programs must use the Faculty Profile Table template that is available on the ACEN website. This is a required template and includes information that is critical for peer evaluators to assess compliance with Standard 2, specifically Criteria 2.1, 2.2, 2.3, 2.6, and 2.10. The table includes a coversheet for identifying the educational and experiential requirements of all agencies, tables for full- and part-time exclusive and shared faculty, and the addenda. Instructions for completion of the Faculty Profile Table are also available on the ACEN website.

When writing to select Criteria in Standard 4, abbreviated syllabi (one or two pages) are required. When required, the abbreviated syllabi should minimally include the following elements:
  • Course name and number;
  • Credit/clock hours related to didactic, skills/laboratory, and clinical;
  • Course-specific prerequisites/corequisites;
  • Course description;
  • Course objectives/outcomes;
  • Methods of evaluation, including information regarding how each contributes to the overall course grade; and
  • Topical outline for course content.
All programs must have a systematic plan of evaluation (SPE) that includes Criterion 6.1, 6.2, 6.3, and 6.4. However, the program faculty may develop and use their own format for the SPE consistent with the expectations outlined in Standard 6. Although a specific template is not required, the ACEN Directors have created two SPE templates (here and here) that are popular with programs across the country. These templates include areas for each of the required components of the SPE.

Finally, instructions sheets for the various ACEN written reports (i.e., the Self-Study Report, the Candidacy Presentation, the Follow-Up Report, and the Focused Visit Report) are available on the ACEN website. The instructions include information about which Standards and Criteria must be addressed for each type of written report.
